This Machu Picchu guided tour packs the whole logistics chain into one day and puts a guide at the centre of it: hotel pickup door to door, a van to Ollantaytambo station, round-trip train to Aguas Calientes, the shuttle bus up to the gate, the entrance ticket and a two-hour guided walk inside the citadel. The day before departure you get a pre-tour briefing at the operator's Cusco office, which is where times, train class and the pickup window are confirmed. Meals and gratuities are not included. After the citadel you come back down to Aguas Calientes with an extra hour there, which is the sensible slot for lunch in a local restaurant. Altitude is a Cusco question rather than a Machu Picchu one. The citadel stands at 2,340 m against 3,399 m in Cusco, so spend a day or two in the city first and treat the descent to the Urubamba valley as the easy part of the trip.
